yes, the user manual is not for elastix. it is for asterisk+zaptel+centos environment. please check the link from:

you can replace the chan_zap.c in asterisk source code , wctdm.c and put opvxa1200.c in zaptel/kernel
compiling zaptel in this way for A1200P/A800P
1) add opvxa1200 in topmodule(TOPDIR_MODULES:=) in Makefile
2) add opvxa1200.o: kernel/wctdm.h kernel/zaptel.h
find the line as below:
ztprovision.o: kernel/zaptel.h
ztmonitor.o: kernel/zaptel.h
opvxa1200.o: kernel/zaptel.h kernel/wctdm.h
ztspeed: CFLAGS=
3) make and make install.
, I do not run. For Elastix, the path is different, I think.

please download the source code for asterisk and zaptel. you can unzip those files under /usr/src/ and edit the makefile under zaptel folder.

please check with zapata.conf, there are many formats asterisk can support.
no matter you are using trixbox or elastix or pure asterisk, all are defined in zapata.conf. of course, asterisk i believe, supports the countries you mentioned, otherwise, asterisk can not be so widely used.
